    What You Should Know: 

    – Neuroglee Health, a leader in value-based dementia care, has announced an expanded collaboration with Mayo Clinic to deliver comprehensive virtual cognitive and dementia care to patients and provide dedicated support for their care partners. 

    – The collaboration builds on their previous work developing the Neuroglee Connect™ solution, which is guided by the HABIT Healthy Action to Benefit Independence & Thinking® program.

    Comprehensive, Virtual Care Model

    Neuroglee’s program enrolls patients with cognitive impairment and dementia for longitudinal virtual care delivered by a specialized team. This team includes advanced practice nurses, licensed social workers, pharmacists, and care navigators, all supervised by neurologists and geriatric psychiatrists. The platform provides 24/7 “high-touch, wraparound support” for patients and their families.

    The whole-person-focused solutions emphasize care partner enablement, in-home care continuity, and avoiding hospital utilization. The care teams aim to improve patient outcomes and experience while controlling costs through proactive symptom management, efficient testing and diagnosis, and remote monitoring.


    Reducing Costs and Improving Outcomes

    The collaboration comes at a critical time for dementia care costs. The average annual per-member cost to Medicare for patients with dementia is $22,000, which is almost three times the cost for members without Alzheimer’s or another form of dementia. Neuroglee’s proactive care model is designed to address the known drivers of high-cost utilization.

    With Neuroglee’s platform, payers and risk-bearing entities could save over $6,300 per member per year. These savings are achieved through early detection and comprehensive longitudinal care, with the model adapting dynamically based on the patient’s diagnosis and cognitive stage. In the early stages, the focus is on patient-centric support, which transitions to care partner-centered enablement as the dementia progresses.
