What You Should Know
- Consumer health data integration platform b.well Connected Health and digital advance care planning (ACP) leader MyDirectives announced an enterprise partnership to integrate advance care planning documents directly alongside longitudinal medical records.
- Represents a major milestone in health data rights: b.well serves as the first consumer-facing organization to integrate directly with MyDirectives’ nationwide registry for a direct patient access use case, expanding beyond traditional clinical facilitator and first-responder lookup portals.
- Connects b.well’s FHIR-based data network and CMS Aligned Network with MyDirectives’ A|D Vault Exchange™, which maintains the largest nationwide registry of advance directives and portable medical orders (including POLST and MOLST forms).
- Directly addresses an acute emergency care data failure: critical end-of-life and acute medical preferences are historically locked away in physical file cabinets, safe deposit boxes, or fragmented legal PDFs, leaving attending emergency clinicians blind to patient wishes during acute trauma or incapacitation.
Unlocking the National Advance Care Planning Registry
MyDirectives operates the largest digital advance care planning registry and repository in the United States, maintaining an interoperability footprint that reaches more than 90 percent of the domestic healthcare ecosystem:
- Massive Query Velocity: The platform’s proprietary A|D Vault Exchange™ registry processes more than 5 million secure advance care planning document queries each week, enabling acute health systems, health plans, and emergency first responders to surface patient preferences in crisis scenarios.
- Comprehensive Document Spectrum: Houses the full clinical taxonomy of end-of-life and crisis documentation, including digital advance directives, uploaded notarized legal forms, mental health advance directives (PADs), and state-specific portable medical orders (POLST, MOLST, POST, and MOST).
- Enterprise Security Standards: Operates under certified compliance with HITRUST Risk-Based 2-year (r2) and SOC 2 Type II frameworks, ensuring that sensitive personal declarations remain protected across multi-point exchange environments.
Historically, MyDirectives integrations focused almost exclusively on provider and emergency access points—either via MyDirectives for Clinicians™ within EHR clinical workflows or through query-and-retrieve integrations for emergency department trauma desks.
The partnership with b.well represents an operational first: b.well is the first consumer-facing digital health organization to integrate directly with MyDirectives’ nationwide registry specifically for a patient-directed access use case.
Delivering Directives via the CMS-Aligned Network
Under the technical integration, advance care planning documents will be ingested through b.well’s FHIR-based platform and made available as a native data type within the consumer’s unified health summary:
- Consumer Health Record Unification: Rather than requiring patients to register for a disparate, single-purpose portal, advance directives will sit alongside longitudinal electronic health records, active prescription lists, diagnostic results, and claims history.
- Distribution Across the CMS-Aligned Network: The data exchange will flow across b.well’s CMS-Aligned Network, which supports real-time FHIR-based bulk exchange, federated identity verification, and granular patient-consent management across more than 300 health plans and 1.8 million healthcare providers.
- Proactive Payer & Provider Alignment: By making treatment preferences accessible before an acute crisis occurs, the partnership supports commercial payers, Medicare Advantage, and Dual-Eligible Special Needs Plans (D-SNPs) seeking to meet CMS health tech ecosystem interoperability mandates and improve Medicare Star Ratings tied to advance care planning documentation.
