Redesign Health Study: 71% of Health Systems Adopt an "Epic-First" AI Purchasing Strategy

    What You Should Know

    • 71% of surveyed health system leaders describe their organization as “Epic-First,” and 80% expect this preference to intensify over the next 3 to 5 years, according to research study from Redesign Health
    • 91% of executives possess “complete” (60%) or “significant” (31%) confidence that Epic will execute its AI roadmap to match best-in-class external solutions.
    • Health systems deploy an average of 57% of their clinical and administrative IS budget to Epic, leaving 43% for external software vendors.
    • 43% of respondents report that AI tools hit service-line or departmental budgets before transitioning to central IT, offering startups a direct entry point via autonomous department leads.
    • 78% of executives require external startups to demonstrate a higher ROI compared to Epic solutions, with 49% demanding a “significantly higher” return to justify deployment.

Beyond the Default EHR: How Health Tech Startups Win in an Epic-First Ecosystem

A primary research study from Redesign Health reveals how platform consolidation around Epic Systems is reshaping software procurement across health systems. The report, titled “The Epic Effect,” surveyed 112 senior healthcare executives—including CEOs, CIOs, CMIOs, and Vice Presidents—at Epic-based health systems to map how health system leaders evaluate third-party software and AI applications within an “Epic-first” ecosystem.

    The Epic-First Baseline

The study highlights a distinct shift toward core EHR platform consolidation:

    • Platform Preference: 71% of respondents describe their organizations as “Epic-first,” prioritizing native Epic modules whenever possible. The remaining 29% evaluate Epic and external vendors on an equal “best-in-breed” basis. Zero respondents reported an inherent baseline preference for third-party vendors.
    • Trajectory: 80% of executives expect their focus on Epic solutions to intensify over the next three to five years.
    • Execution Confidence: 91% of leaders express high confidence (60% “complete confidence,” 31% “significant confidence”) that Epic will execute its AI roadmap to match best-in-class external alternatives.
    • Budget Allocation: Health systems allocate an average of 57% of their IS software budget to Epic and 43% to external vendors.

    Factors Driving Epic Selection

When health systems select Epic over external alternatives, executives cite specific operational advantages:

    • Ease of Integration: 50%
    • Superior Capabilities: 48%
    • “Good Enough” Functionality: 48%
    • Total Cost-Effectiveness: 47%
    • Minimizing Workflow Complexity: 33%

    Strategic Entry Points for Startups

Despite strong platform alignment with Epic, health system leaders identify clear functional domains where third-party software vendors can successfully compete:

    • Clinician-Facing AI & Ambient Scribing: 48%
    • Imaging AI & Diagnostics: 44%
    • Patient AI Assistants & Engagement: 40%
    • Care Access & Scheduling: 28%
    • Discharge & Care Transitions: 25%
    • Revenue Cycle AI: 24%

Key Buying Drivers for External Vendors

When external startups win contracts over Epic modules, ease of integration is cited by 63% of executives as the primary differentiator, closely followed by faster time-to-value (59%) and specialized capabilities that extend beyond native EHR features.

Additionally, 43% of respondents note that emerging AI applications hit departmental or service-line budgets first, where 52% of department heads maintain independent purchasing authority—offering an entry pathway for specialized software before enterprise-wide IT review.
